FDA OKs First Drug–Eluting Stents For Use in MI

Drug–eluting stents (DES) have been used in diabetics and/or acute MI as off–label use up till now. With the FDA approving specific stents in these conditions, its legal implications are many.

Drug–eluting stents are preferred to bare–metal stents because they reduce the incidence of restenosis and target vessel revascularization. Similar outcomes have also been shown in patients with acute ST elevation myocardial infarction (STEMI) undergoing primary PCI.

Since patients with an acute MI (within 48 to 72 hours) were excluded from the pivotal SIRIUS and TAXUS IV trials, primary PCI is considered an “off–label” use in the United States for all DES.

In the HORIZONS AMI trial, 3006 patients with STEMI patients were randomly assigned to either paclitaxel–eluting Taxus stents (PES) or BMS in a 3:1 ratio. The 12 month rates of ischemia–driven target vessel revascularization were significantly lower after treatment with PES a difference that was entirely due to a lower rate of target lesion revascularization.

Based on the results HORIZONS–AMI trial, two drug–eluting stents from Boston Scientific have become the FIRST to be approved by the US FDA for treating patients with acute myocardial infarction.

  • Ion: Paclitaxel–eluting platinum chromium
  • Taxus Liberté – Paclitaxel–eluting

The Ion stent, which employs polymer and drug elution similar to that of the Taxus, differs with respect to its platinum chromium metal alloy structure and design, which allow for thinner struts and more flexibility within the coronary arteries.

Diabetic stent FDA approved

US FDA has also approved Medtronic Resolute Integrity drug-eluting stent, which is the first stent the agency has approved to treat coronary artery diseases in diabetics.

Diabetics suffering from coronary artery disease represent 30% of the nearly 1 million PCI procedures performed yearly in the U.S and 1.2 lakhs in India. Such procedures are inherently more difficult in patients with diabetes because their coronary arteries are narrower than those not afflicted by the condition.

The Resolute Integrity DES offers several notable benefits, starting with outstanding deliverability, which means it is exceptionally easy to navigate the stent on the delivery system through the coronary vasculature to the narrowed arterial segment that requires treatment.

Holashtak beginning 1st March 2012 is name used to refer to the eight days prior to Holi festival and is considered inauspicious in mythology.

In North India, Holashtak begins on the Phalgun Shukla Ashtami (eighth day during the waxing phase of moon in Falgun month) and ends on the Phalgun Purnima day (full moon day). The Purnima day is observed as Holika Dahan.

It is considered inauspicious to perform auspicious ceremonies during the period like marriage, housewarming or any of the sanskars. People also avoid starting of new business etc. The day is considered ideal for performing Dhaan – alms giving.

The difference between Holi and Diwali is that holi is observed in Uttarayana (the period with positive state of mind) and Diwali in Dakshinayana (the period with negative state of mind).

 In Holi we try making up with our enemies and in Diwali we only redefine our existing friendship.

 Holi means burning negativity in our mind and in the mind of our enemies. This can only happen in a period of positive state of mind (first six months of the year). The process of removing negativity requires multiple attempts. As per mythology it requires eight days ( or eight attempts) to remove the negative attitude of a person and hence the word Holasthak.

During the period one is trying to wash out the negativity, it is best to avoid calling people for community functions as in that case you will need to call the one with whom you are trying to patch up and the same may not be in sound frame of mind with you and you may end up with an uncomfortable situation.

Pre-treatment with propranolol a beta blocker can block the cardiovascular effects of marijuana. It can prevent the learning impairment and, to a lesser degree, the characteristic subjective experience.

Marijuana is known to induce typical subjective state (“high”) with marked increases in HR, BP and conjunctival infection. It impairs performance on a learning test without significantly affecting attention.

Bhang is a traditional Indian beverage made of cannabis mixed with various herbs and spices, which has been popular inIndiafor ages.

Bhang is a less powerful preparation than Ganja, which is prepared from flowering plants for smoking and eating.

Charas, more potent than either Bhang or Ganja, is made from cannabis flower tops harvested at full bloom.

Charas which is thickened with sticky resin is nearly as potent as the concentrated cannabis resin preparations called hashish.

Eating less calories can delay primary aging. “The less you live the more you live” has been a Vedic saying. Calorie restriction has previously been shown to slow aging and increase lifespan in small mammals.

Adhering to a calorie-restricted diet over the long term ameliorates the normal age-related decline in diastolic (relaxation) function of the heart in healthy, non-obese adults. The cardiac beneficial effects of caloric restriction are mediated by reductions in blood pressure, systemic inflammation, and myocardial (heart muscle) fibrosis as per a study published in the Journal of the American College of Cardiology.

Dr. Fontana, fromWashingtonUniversityinSt. Louis, performed Doppler Echocardiography (ultrasound of the heart) and measured inflammatory markers in 25 healthy adults who followed a severely calorie-restricted, nutritionally balanced diet for an average of 6.5 years. Results were compared with those from 25 matched control subjects consuming a typical Western diet.

The calorie-restricted diet consisted of roughly 1,671 kcal per day derived approximately 23% protein, 49% complex carbohydrates, and 28% fat (including 6% saturated fat). The Western diet consisted of roughly 2,445 kcal per day made up of about 17% protein, 52% carbohydrates, and 31% fat (11% saturated fat). The calorie-restricted diet included at least 100% of the recommended daily intake for all nutrients, and it was lower in salt than the Western diet.

People who followed a calorie-restricted diet but with optimal nutrition had a younger heart in terms of diastolic functions, a marker of primary aging. Mean systolic (upper) and diastolic (lower) blood pressures were significantly lower in the calorie-restricted group (102/61 vs. 131/83 mm Hg), as were levels of the inflammatory markers C-reactive protein, TNF-α, and TGF-β1.

 ‘Eat less to live more’ has its origin in Vedas. The Ayurvedic text from Atharvaveda defines the limit of food intake in a particular meal. It says one should not eat more than what can be accommodated in an outstretched palm.

 According to the Jain philosophy there are 12 types of penance. Two of them are Unodari andAnshan(fast). Unodari is for those people who cannot fast and it is eating less than what is needed to satisfy one’s hunger. Deepak Chopra in his book perfect health says that one should eat to a scale of 7 from a hunger scale of 10.

Yogashastra says that one who eats one meal in a day is yogi; one who eats two meals is bhogi and one who eats three meals a day is a rogi. The common Sanskrit saying is “ekabhattam cha bhoyanam” which means if one takes only one meal a day then there will be no illnesses.

 In Bhagavad Gita also the law of moderation is defined. Lord Krishna says to Arjuna (6.17) “Yukaharaviharasya yuktachestasya karmasu; Yuktasvapnavabodhasya yoga bhavati duhkhaha”. The shloka states that “for the one, whose diet and movements are balanced, who does his actions in a proper manner, whose hour of sleeping and waking up are regular, and who follows the path of meditation is the destroyer of pain or unhappiness.”

 Controlled diet conserves one’s energy. Samana vayu, the vital energy produced around the naval, remains in balance if one eats less. Over eating reduces this energy.

A recent study published in June 2011 Edition of the Journal of Diabotologia also has shown that type-2 diabetes can be reversed by reducing the dietary intake. Up till now, type-2 diabetes was being regarded as inevitably progressive with irreversible beta cell ( cells in the pancreas responsible for controlling sugar) failure.  The authors fromUKstudied 11 patients with type-2 diabetes with a mean age of 50 years and a mean body mass index of 34 and evaluated them for 8 weeks on 600 calorie per day diet. Within a week of restricted diet, fasting sugar normalized in the diabetic group. All available tests to check for beta cell dysfunction normalized within eight weeks. There was a normalization of insulin resistance and other biochemical abnormalities like insulin suppression, hepatic glucose and hepatic triglyceride content and the first phase insulin resistance.

Both bariatric surgery (surgery done for obesity) diet and “non-scalpel bariatric surgery” diets can cure diabetes and reverse obesity related ageing. The diet includes consuming less than 80 gm or 80 ml of caloric food at one go with an average diet of less than 1200 k. cal per day. The diet includes zero trans fat, zero refined carbohydrates and only one complex carbohydrate meal a day.

Another way of eating less is “vritti samkshepa”. It involves limited number of items eaten and not to eat same particular items on particular days. Traditional Indians “vrata” (fast) also are based on this type of penance. Many “vrata” restricts eating particular items on that day. Rice is not eaten on ekadashi day.

Another way of eating less is penance of “raso parityaga”, which means avoiding of certain tastes of food. One should not eat items with all taste on all days. The Friday fast of Santoshi Maa involves not eating sour food on that particular day.

Many religions insist on not eating after the sunset. One of the phrases in Yoga Shastra is “astangate divanathe” which means that the digestive system becomes inactive after the sunset. It is also the philosophy of Ayurveda. As sun is the greatest source of energy, body’s vital, physical and digestion remains active in presence of sun. While Jainism talks about not eating at all after sun set, Ayurveda says one should have dinner lighter than lunch and to avoid artificial food as well as semi digestive foods in the night. Eating curd has been prohibited in Ayurveda after sunset. People living in South also do not eat fermented food items like idli, dosa, etc. after sunset.

According to Ayurvedic philosophy any food that is eaten after sunset does not get properly transformed into juices. Such food, therefore, are not helpful for health as they are not converted into energy.

In modern medicine over eating now is classified as a type of malnutrition. Obesity, high blood pressure, heart disease, high cholesterol, cancer, gall stones, etc. are as a result of over eating.

Car exhaust and other air pollution, even at levels considered safe by federal regulations, may substantially increase the risk of a stroke, a research team from Beth Israel Deaconess Medical Center has found.

After reviewing the medical records of more than 1,700 stroke patients in the Boston area over 10 years, the researchers found a 34 percent increase in the risk of ischemic strokes on days with moderate air quality compared with days when the air was rated good by the US Environmental Protection Agency.

A lifetime’s exposure to air pollution may contribute to mental decline in older women,” according to the study published online Feb. 13 in the Archives of Internal Medicine.

Both exposure to fine particulate matter air pollution — less than 2.5 micrometers in diameter, or about 1/30th of the diameter of a human hair — and coarse particulate matter — between 2.5 and 10 micrometers in diameter — were associated with mental declines in women.

A second study published in the same journal “found that more people were admitted to aBostonhospital for ischemic stroke on days when levels of fine particulate air pollution were high.

The second study “found that short-term exposure to fine particulate matter — even at levels allowed by the EPA – can increase the risk of ischemic stroke.

The relationship between higher particulate levels and increased risk of stroke was linear, strongest within 12 hours of exposure, and was seen among patients with strokes caused by large-artery atherosclerosis or small-vessel occlusion but not cardioembolism. [AMA News]

The true silence is the silence between the thoughts and represents the true self, consciousness or the soul. It is a web of energized information ready to take all provided there is a right intent. The process of achieving silence is what meditation is.

Observing silence is another way of getting benefits of meditation. Many yogis in the past have recommended and observed silence now and then. Mahatma Gandhi used to spend one day of each week in silence. He believed that abstaining from speaking brought him inner peace and happiness. All such days he used to communicated with others only by writing on paper.

Hindu principles also talks about a correlation between mauna (silence) and shanty (harmony). Mauna Ekadashi is a ritual followed traditionally in our country. On this day the person is not supposed to speak at all and keep complete silence through out day & night. It gives immense peace to the mind and strength to the body. In Jainism this ritual has a lot of importance. Nimith was a great saint in Jainism who long ago asked all Jains to observe this vrata. Some people recommend that on every ekadashi one should observe silence if not the whole day but for few hours in a day.

Deepak Chopra in his book 7 Laws of Spiritual Success talks in great detail about the importance of observing silence in day today life. He recommends everyone should observe silence for 20 minutes everyday. Silence helps redirecting our imagination towards self from the outer atmosphere. Even Swami Sivananda in his teaches recommend to observe mauna daily for 2 hours for ekadashi, take milk and fruits everyday, study daily one chapter of Bhagwad Gita, do reguarl charity and donate one-tenth of the income in the welfare of the society. Ekadashi is the 11th day of Hindu lunar fortnight. Ekadashi is the day of celebration occurring twice a month, meant for meditation and increasing soul consciousness. Vinoba Bhave was the great sage of our contry who is known for this bhoodaan movement. He was a great advocator and practical preacher of mauna vrata.

Mauna means silence and vrata means bow hence mauna vrata means bow of silence. Mauna was practiced by saints to end enemity and recoup their enemity. Prolonged silence as the form of silence is observed by the rishi munis involved for prolonged periods of silence. Silence is a source of all that exists. Silence is where consciouns dwells. There is no religious tradition which does not talk about silence. It breaks a out world communication and forces a dialogue towards inner communication that one reason why all prayer, meditation and worship or any other practice whether we attune our mains to the spiritual consciousness within are done in silence.  After the death of a person it is a practice to observe silence for two minutes. The immediate benefits is that it saves a tremendous amount of energy.

Silence is cessation of both sensory and mental activity. It is like having a still mind and listening to the inner mind. Behind this screen of our internal dialogue is the silence of spirit. Meditation is the combination of observing silence and the art of observation.

1. Stay fit

Stretching and strengthening both your back and abdominal muscles is important. A stretching and strengthening regimen should target the back, abdominal, and buttock muscles.

Bicycling (either stationary or regular), swimming, and walking lead the list of low–risk, high–benefit activities for most people’s backs. All are low– or minimal–impact exercises that strengthen muscles which support the back. None involve awkward or stressful actions that are detrimental to back muscles.

Sports and activities such as football, tennis, gymnastics, wrestling, weight lifting, rowing (crew), running, aerobic dance, and ballet involve a relatively high risk for back injury because of the extension, lifting, or impacts involved. Other unnatural motions that could induce pain include back arching (during gymnastics and diving), twisting (while hitting a golf ball, swinging at a baseball, or bowling), vertical jolting (while riding a horse), and stretching your legs strenuously (when hiking or when balancing a sailboat during a race).

2. Maintain a healthy weight

The heavier you are, the greater the load your spine must carry. To make matters worse, if the bulk of your weight comes in the form of abdominal fat, rather than muscle, your center of gravity can shift forward — a condition that puts added pressure on your back.

3. Kick the habit

Nicotine in cigarettes contributes to low back pain in two ways. First, nicotine hampers the flow of blood to the vertebrae and disks. This impairs their function and may trigger a bout of back pain. Second, smokers tend to lose bone faster than nonsmokers, putting them at greater risk for osteoporosis, another common cause of back pain.

4. Lighten your load

A survey by the American Academy of Orthopaedic Surgeons found that nearly 60% of the doctors responding had treated school–age patients complaining of back and shoulder pain caused by heavy backpacks.

1. Use both of the pack’s straps instead of slinging one strap over a shoulder.
2. Carry only the essentials
3. Lighten your load whenever possible.
4. Opt for backpacks that have different–sized compartments to help distribute weight evenly.
5. Look for wide, padded straps and a padded back.
6. When carrying a heavy load, put the heaviest items as close as possible to the center of the back
7. Use the hip strap for support.
8. For very heavy loads, use a backpack with wheels.
9. Bend from your knees when picking up your pack.

5. Develop back–healthy habits

1. While standing to perform ordinary tasks like ironing or folding laundry, keep one foot on a small step stool.
2. Don’t remain sitting or standing in the same position for too long. Stretch, shift your position, or take a short walk when you can.
3. When bending from the waist, always use your hands to support yourself.
4. When sitting, keep your knees a bit higher than your hips and bend them at a 90–degree angle. Sit with your feet comfortably on the floor. If your feet don’t reach the floor, put a book or a small stool under them.
5. Because vacuuming can take a toll on your back, tackle rooms in chunks, spending no more than five to 10 minutes at a time doing this task.

Everyone behaves according to his or her needs. Every behavior is justified medically as every action is performed according to one’s own psyche, which in turn depends upon the needs. The most primitive needs of a person are that of food, shelter and procreation. Most thefts, minor criminal acts, eve teasing, rapes, etc are to fulfill these needs. These needs satisfy the physical body.

Those people whose physical needs are satisfied are controlled by the mental needs, which can be at the level of mind, intellect or ego. Egoistic needs are focused towards attention, status, and power grabbing. Most political decisions at the state, national or international levels are taken to fulfill these “ego based desires”. The action of George Bush against Iraqin the recent era and that of Hitler in the past are classical examples.

Those who have conquered the mental needs by controlling the mind are controlled by spiritual needs. Everyone with a balanced state of mind needs affection, appreciation, acceptance and inner happiness. Spiritual fulfillment of needs based on non-violence.

There are very spiritual leaders living in the world today and hence most behaviours today either are at the level of physical fulfillment of desires or at the level of mental fulfillment.

Most crimes are committed and most criminals are trained using these principles. Individuals are chosen from the general public who are either gullible (hypnotizable) or are frustrated because of non-fulfillment of their needs. Frustrated people are first addicted to food, women, wine, drugs or power and then made to enter into criminal activities or brainwashed to such an extent that except for the goal for which they are hypnotized, nothing else exists for them.

It is a well-known scientific fact that in a state of deep mental relaxation the mind becomes suggestive. Hypnotizable people are the ones who can be made to go into mental relaxation with simple mental exercises. When their restful mind is bombarded with suggestions positive or negative, the intended suggestions get attached to their consciousness and the whole body’s biochemistry changes and acts in a way to fulfill that intent.

To reduce violence in the community, one needs to reduce violence in a family or in an individual mind. It is the violence in the collective consciousness of the people, which allows such violent attacks in the community.

In a recent survey by the media, it was found that 80% of the people agreed that for attaining a promotion or getting a seat in a medical college, conversion to schedule caste or schedule tribe is justifiable.

According to the Bhagwad Gita, there are three main gateways to misery and they are lust, greed and ego. If they are satisfied, more lust, greed or ego follows. Unsatisfied lust ends up in anger, delusion and misery; unsatisfied greed leads to attachment and resultant negative actions; and finally, unsatisfied ego leads to violent acts.

Women more often have no chest pain with a heart attack and have a greater risk of dying in hospital than men do.

Almost 40% more women had no chest pain at diagnosis, and they had a 42% higher in hospital mortality, researchers reported in the Feb. 22/29 issue of the Journal of the American Medical Association.

The youngest women with MI were most likely to have no chest pain and also had the highest mortality.

Absence of pain is associated with delayed diagnosis, less aggressive treatment, and a twofold higher short-term mortality.

Young female heart attack patients might have an increased mortality risk, and absence of chest pain or discomfort might contribute to that risk.

Women tend to be older at hospitalization for acute heart attack.

 

 

The best description of four types of devotees comes from Bhagvat Gita where Krishan says to Arjuna (7.16)

“Chaturvidha bhajante mam janah sukrtino rjuna.
Arto jiijnasurarthasthi jnani cha bharatasabha:”

This means that there are four types of beings who worship me: those who are unhappy, those who are desirous to have knowledge, those who desire worldly objects, and those who have acquired knowledge. In the next shloka,Krishnasays that among them only those who have acquired knowledge (wisdom) are the best because they are always engrossed in worshipping me. He said those who have a knowledge love me, and I love them.

In any seminar you can find the same four types of delegates. Some who come have an interest in that particular seminar as they have been facing a difficulty related to that subject; some come because they are desirous to have knowledge in every subject; others want to attend it to get sutras to earn money, and lastly is a group who has knowledge in the subject and wants to revise it or have a lively interaction in it.

In most seminars one will find a group of persons who are very fond of asking questions. They will ask question whether or not they understand the question. They will be present in every seminar of that subject and ask the same questions. Other will be who will know the answers but want to enjoy the scenario by asking unnecessary questions (to enjoy the worldly questions).

Rabbi Dovid Rosenfeld, a known scholar has classified four types of students in a different way. He describes them as a sponge, a funnel, a strainer, and a sifter. The sponge absorbs everything. The funnel brings in on one side and brings out on the other. The strainer lets out the wine and retains the lees. The sieve lets out the flour dust and retains the fine flour.”

This classification is according to the ability to retain the knowledge taught to them. The sponge retains everything, but is unable to distinguish between correct and incorrect points or between significant and insignificant ones. He is devoid of Viveka, the power of discrimination.

The funnel is the one for whom information goes in one ear and out the other. He has no focus on the lecture. His hearing and the mind are in different directions.

The strainer discards the wine – the significant material, and retains the lees – the incorrect or insignificant points. He’s the sort who remembers all sorts of trivial or useless details of the material he studied. Most students try to remember the foot notes of a book and forget the common things.

Finally, the sieve retains the fine flour – the significant material, and discards the dust – the inconsequential details. He is the one who understands the lecture by its main five points and remembers them in the form of sutras.

One should not simply remember and regurgitate everything heard. It is like parrotification (like a parrot). One should not only hear but also listen to it.

One more way of classifying the students is on the principle ‘ suno – samjho-jano–karo ” given by the sages. This means hearing, listening, knowing and wisdom.

Hearing is like funnel where your mind is not listening. You hear from one ear and pass out of the other. These students can be seen to be absorbed in some other world during the lecture.

Listening is one step ahead of hearing. It requires reasoning and understanding its meaning.

Knowing is one step further and requires not only understanding its meaning but its usefulness in one’s own life.

Wisdom is the final step of learning when one starts implementing it in his life effortlessly or has stitched the knowledge to his consciousness.

If you want to become the best student remember the following mantra:

  • What ever you listen, ask the following four questions to yourself
  • Is it the truth? (Is it evidence and fact-based?)
  • Is it necessary to know this?
  • Will it be useful to me in the future?
  • Will it be of any help to others through me?
  • If the answers to these are ‘yes’, this is the right knowledge.

Agni Yoga (1929) – 105 also described, four types of disciples. Some follow the Indications of the Teacher and ascend in proper order. Others, behind the Teacher’s back, follow the Indications to excess and thereby often harm themselves. Others, in the Teacher’s absence, take occasion to prattle and thereby destroy their path. Others, behind His back, criticize the Teacher and betray Him. Dreadful is the destiny of these last two kinds!

Nine out of 10 adults in the US consume more sodium than is recommended. The findings are laid out in the Centers for Disease Control and Prevention’s Morbidity and Mortality Weekly Report on February 7, 2012.

The estimates of salt intake from 7227 participants in the National Health and Nutrition Survey 2007–2008 show that the mean daily intake of sodium is 3266 mg. Daily guidelines for maximum sodium consumption are 2300 mg.

44% of sodium consumed came from 10 food categories: bread, cold cuts and cured meats, pizza, poultry, soups, sandwiches, cheese, pasta mixed dishes, meat mixed dishes, and savory snacks. More than 70% of the sodium eaten came from goods obtained at a store.

Reducing the sodium content of the 10 leading sources by one-quarter would reduce total dietary sodium by more than 10%, preventing an estimated 28 000 deaths.

It’s the upper part of the plants, the leaves, flowers and fruits, which is worshipped as sacred and offered to pooja and GOD. As per Bhagwad Gita they have satvik properties. Roots of any plant are tamasic and not offered to pooja or eaten during pooja days. Same is true for the stem of the plants which have rajasic properties.

Fresh and live fruits have the same spirit and life force as in the human beings and are considered sacred the same way as is any human being.

Human life also depends on plants and trees. They give us the vital factors that make life possible on earth: food, oxygen, clothing, shelter, medicines etc.

Indians scriptures talk about planting minimum ten trees. We are also urged to apologise to a plant or tree before cutting it to avoid incurring a specific sin named soona.

Caffeine intake associated with coffee consumption has been linked to a reduced risk for clinical and pathologic progression of liver fibrosis in patients with hepatitis C virus, and coffee was also found to reduce the risk for liver enzyme elevations in patients with alcoholic liver disease. Drinking caffeinated coffee protects against liver fibrosis in patients with NAFLD, according to research published in the February issue of Hepatology by Jeffrey C. Molloy, MD, from the Division of Gastroenterology and Hepatology,WilfordHallMedicalCenter, Lackland Air Force Base,Texas.

Moderate coffee consumption may be a benign adjunct to the comprehensive management of patients with [NASH]. Ne should however avoid risk factors, such as obesity, high alcoholic intake, and viral hepatitis. [Hepatology. 2012;55:429-436]
