  • Seizure is a sudden change in behavior that is the consequence of brain dysfunction.
  • Approximately 0.5-1% of the population has epilepsy.
  • Some seizures are provoked, i.e., that occur in the metabolic derangement, drug or alcohol withdrawal and in situations like acute paralysis or acute encephalitis. Such patients are not considered to have epilepsy because these seizures would not recur in the absence of the provocation.
  • Less than 50% of epilepsy cases will have an identifiable cause such as head trauma, brain tumor, paralysis, infection, brain malformation, etc.
  • Having one seizure does not always mean that the patient would always get a seizure.
  • One episode of seizure may not require treatment.
  • Hospitalization is required in the first seizure only if it is associated with prolonged post-seizure altered level of consciousness.
  • Patients with unprovoked seizure may not be allowed to drive for some time.
  • In children, seizure can occur with high grade fever.
  • In adults, the first episode of seizure may be due to worms in the brain. In such a situation, it may be necessary to do an MRI test.
  • A patient with seizure can get married, live a normal life and produce children.
  • It is a misnomer that during a fit, you need to make the person smell a shoe.
  • During a seizure, use a spoon instead to prevent tongue bite. Never put the fingers inside the mouth of the patient as you could be bitten.
  • A patient with epilepsy fall will have stiffness in the body; on the contrary, a patient with cardiac loss of consciousness will fall loose.
  • A seizure that lasts for more than 5-10 minutes requires specialized attention.

In Paralysis, Act Fast

Brain attack should be tackled like a heart attack. As time is brain, a patient with suspected paralysis/stroke or brain attack should be shifted to hospital at the earliest and given a clot dissolving therapy if the CT scan is negative for brain hemorrhage. Prevention for paralysis is the same as prevention for heart attack. All patients with paralysis should be investigated for underlying heart disease and all patients with heart diseases should undergo testing to detect blockages in the neck artery, which can cause future paralysis.

Facts

  • Brain haemorrhage should be ruled out as soon as possible.
  • Obtain emergent brain imaging (with CT or MRI) and other important laboratory studies, including cardiac monitoring during the first 24 hours after the onset of ischemic stroke.
  • Check glucose and correct high or low sugar. If the blood sugar is over 180 mg/dL, start insulin.
  • Maintain normothermia for at least the first several days after an acute stroke.
  • For patients with acute ischemic stroke who are not treated with thrombolytic therapy, treat high blood pressure only if the hypertension is extreme (systolic blood pressure >220 mmHg or diastolic blood pressure >120 mmHg), or if the patient has another clear indication (active ischemic coronary disease, heart failure, aortic dissection, hypertensive encephalopathy, acute renal failure, or pre–eclampsia/eclampsia).
  • For patients with acute ischemic stroke who will receive thrombolytic therapy, antihypertensive treatment is recommended so that systolic blood pressure is 185 mmHg and diastolic blood pressure is 110 mmHg.
  • Antithrombotic therapy should be initiated within 48 hours of stroke onset.
  • For patients receiving statin therapy prior to stroke onset, it should be continued.

Meditation and not medication should be the first line of treatment for most lifestyle disorders. The very fact that our body has a receptor for each and every drug means it has the capacity to produce that drug. God never made these receptors for pharmacological agents or drugs. The key lies in achieving the undisturbed state of consciousness, which can be obtained by either controlling the disturbed state of mind or bypassing it by using the mantra. The subject of spiritual medicine should be included in schools, colleges and medical sciences.Confession and communication are two easy modes to control a disturbed state of mind. As darkness is absence of light, negative thoughts are absence of positive thoughts. To reduce negative thoughts, one should inculcate positive thoughts, actions and behaviors. One cannot hate a stranger. One can only hate a person whom he or she has loved. Hatred is therefore withdrawal of love, and it can only be removed by bringing the love back. By the process of meditation, it has been shown that diseases like diabetes, high blood pressure, heart disease, paralysis, asthma and acid-peptic disease can be kept under control without or with minimal medicines.

A drug caffeinol containing caffeine and alcohol may help stroke patients recover.

In a small study at Texas Health Science Center in Houston, 60% of stroke patients who were given the drug, had no or minimal disability when they were discharged from the hospital. In contrast, only 26% of stroke survivors given standard therapy with tissue plasminogen activator, or tPA, fared that well.

Caffeinol contains about as much caffeine as 5 to 7 cups of good, strong New Orleans coffee and the equivalent of two shots of alcohol.

The study involved 100 people who had suffered an ischemic stroke. All received intravenous tPA; 10 were also given an infusion of caffeinol. Caffeinol allows cells to tolerate reduced blood flow longer, thereby giving tPA a longer opportunity to do its action.

Will these findings be applicable to heart attack? Only time will tell as heart attack treatment is also done with tPA.

Uncontrolled blood pressure can lead to heart attacks, paralysis and heart failure. Most such attacks occur in the early morning hours. Pulse, blood pressure and thickening of platelets are all higher in the early morning hours.  Controlling early morning blood pressure can reduce cardiovascular mortality. According to a study published in the Journal of the American Society of Nephrology, among patients with chronic kidney disease and high blood pressure, taking at least one antihypertensive drug at bedtime significantly improves blood pressure control, with an associated decrease in risk for cardiovascular events.  The study included 661 patients with chronic kidney disease who were randomly assigned either to take all prescribed anti BP drugs on awakening or to take at least one of them at bedtime. Patients were followed for a median of 5.4 years; during that time, patients who took at least 1 BP–lowering drug at bedtime had approximately one third of the cardiac risk compared with those who took all medications on awakening. A similar significant reduction in cardiac deaths, heart attacks and paralysis was noted with bedtime dosing. Patients taking their medications at bedtime also had a significantly lower mean BP while sleeping. For each 5 mmHg decrease in mean sleep–time systolic upper BP, there was a 14% reduction in the risk for cardiovascular events during follow–up. Potential explanation for the benefit of night time treatment may be associated with the effect of night time treatment on urinary albumin excretion levels. Urinary albumin excretion is significantly reduced after bedtime, but not morning, treatment.

This sutra from Bible has a very deep significance in day to day life. The truth is ever lasting and always ends up in internal happiness and self realization. And in the long run it always gives you happiness and an all win situation. A lying tongue on the contrary will only give you a momentary pleasure but will lead to or create some difficulty later in life. Spoken words cannot come back just as in the case of a released arrow from the bow. Once lost one cannot get back his youth virginity or respect. Similarly spoken bad words cannot be taken back and once spoken will create negative waves in the other persons on whom they were spoken mind which will persist as repressed thoughts or memory in the people s mind forever. Such bad memories will keep on coming back in the person s mind causing damage to the personal relationships. A spoken word is a karmic expression. For every karmic action there is an opposite and equal reaction. For every negative karmic action one has to pay the debt either now or in future. The law of karma says that every debt has to be paid. It is always better to avoid negative language both in spoken words as well as in the mind. The yoga sutras of Patanjali describe thinking speaking or doing anything wrong as having the same karmic significance. We should not only purify ourselves in actions and spoken words but also in the mind. If a person keeps negative thoughts in the mind sooner or later the same will be reflected to the outside world. The momentary pleasure which one gets by lying has no spiritual significance as it only satisfies your ego sense or makes you attached to any of the five senses. The transient pleasure experienced by the body stimulates a chain of reactions consisting of action memory and desire leading to action again which will only intensify the greed attachments. In the Mahabharata Lord Krishna has given only two examples which work as an exception to such a situation. Any truth which harms others may not be spoken and any lie which does not harm anyone but benefits a few may be spoken. Truth is the opposite of doubt and it is always better to clear all the doubts from the mind as any repressed doubts can end up into causation of heart attack paralysis and cancer. Truth also means taking conscious based decisions as the consciousness will never lie. While taking any decision always ask oneself Is it the truth Is it necessary And will it bring happiness to me and the people around Lord Krishna is also described as Satchitanand which only indicates qualities like truthfulness conscious based decisions and internal happiness. Truthfulness has to be practiced for over a period of time and made a part and parcel of your daily life. • People with pre diabetes are at increased risk for developing type 2 diabetes, heart disease and Paralysis.

• More than 25% people have pre diabetes (blood sugar level more than 90 but below 126 mg/dL) but only 4% know it needs attention.

• Diabetes is not just controlling blood sugar. Controlling BP (<120/80) and LDL bad cholesterol (<70) is equally important.

• Smokers face 44% increased risk of diabetes when compared to nonsmokers (Swiss researchers JAMA)

• Advancing age, diabetes, high BP, obesity, smoking, drugs, heart diseases, alcohol, cycling for > 3 hours/week can cause erectile dysfunction.

• Each 1 mg increase in serum uric acid increases the risk of developing diabetes by 18%.

• Keep your fasting sugar, lower BP, bad cholesterol, abdominal girth all below 80.

Amongst celebrities why do only cine stars commit suicide and not cricketers or politicians: What can be done to prevent them?

The death of Jiah Khan, the film actress, reopens up the debate on cine star celebrity deaths. Why do so many suicides occur in the film Industry? Despite similar failure rates, there are no suicides in politics or in cricket.

In cricket, we see cricketers under stress, they even lose their place in the team for years and yet they do not go in depression. The reasons may be the presence of a counselor in every team and opportunities to play in domestic games.

Will the film industry come out with guidelines for suicide and stress prevention?

We have seen cases of Raj Kiran and Parveen Babi (56) who were reported missing for years and nobody bothered. The same never happens in cricket. Parveen Babi was later found dead in her flat.

In the past, many (eight) cine celebrities have committed suicides.  These include Guru Dutt (39), Divya Bharti (19), Nafisa Joseph (26), Kuljeet Randhwa (30), Kunal Singh (31), Silk Smitha (35), Vijay Banerjee (37) and now Jiah Khan (25).

So is celebrity status a risk factor for suicide?

Suicides are also common in medical profession but not in nursing profession. They are hardly seen in cricketing celebrities.

Lifestyle disorders, including heart attack, diabetes, paralysis, liver disease, kidney disease and cancer are still the commonest causes of death amongst celebrities.

  • KL Sehgal (42), Geeta Dutt (41) and Meena Kumari (39) died of alcohol-related liver diseases.
  • 12 celebrities died of cancer: Nazia Hassan 35 (lung cancer), Nargis 51 and Jatin Kanakia 46 (cancer pancreas), Jennifer  Kapoor 51, Nutan 54, Rehman 58 (throat cancer), Rajender Kumar 69, Firoze Khan 69, Prithvi Raj Kapoor 70, Mac Mohan 72 (lung cancer), Shamim Desai (stomach cancer), Sujit Kumar 75.
  • Both Jagjeet Singh 70 and Amrish Puri 72 died of brain hemorrhage.
  • Actors who died because of heart disease: Madhubala 36 (hole in the heart), Bob Cristo 72 (rupture of heart); heart attack: Simple Kapadia 51, Mohd. Rafi 55, Mohan Gokhle 45, Sanjeev Kumar 47, Vivek Shauq 47, Shafi Inamdar 47, Nirmal Pandey 48, Amzad Khan 51, Mukesh 53, RD Burman 54, Kishore Kumar 58, Ravi Baswani 63, Prem Nath 65, Naveen Nishchol 65, Hemant Kumar 69, Suchitra Mitra 86, Dev Anand 88, MF Hussain 85, Vinod Mehra 45, Sunil Dutt 75, Abir Goswami 37, Surinder Kapoor 85, Praveen Babi 55 (diabetes and heart disease) and Rituparno Ghosh 50.
  • Mazhar Khan 45 and Shammi Kapoor 79 died due to kidney failure.
  • Only three died of infections: Geeta Bali 36 due to small pox, Bhimsen Joshi 89 and Jagmohan Mundra 62 due to pneumonia.
  • Only two celebrities were murdered: Gulshan Kumar 41 and Priya Rajvansh 63.
  • There was only one pregnancy-related death: Smita Patil 31.
  • Two died of prolonged illness: Mani Kaul 66 and Goga Kapoor 70.
  • One died of multi organ failure: Bhupen Hazarika 85.

Out of 61 celebrity deaths collected so far,  25 died of heart disease, 12 died of cancer, 3 died of liver disease, 2 of brain attack, 2 with prolonged illness, one pregnancy-related, 3 infections, 8 suicides, 2, kidney failures, 2 murders and one multiorgan failure.

On the other hand, amongst politicians a large number of them have died of unnatural deaths but there are no suicides. The number one cause of death however remains heart attacks or lifestyle illnesses.

Politicians who have died in air crashes include Netaji Subhash Chander Bose (alleged to have died in a plane crash), Homi J. Bhaba, S. Mohan Kr. Mangalam, Sanjay Gandhi, Ashok Birla, Surendra Nath, Madhav Rao Scindia, GMC Balyogi, OP Jindal, YSR Reddy and Dorjee Khandu. Three died in road accidents, Rajesh Pilot, Sahib Singh Verma and Giani Zail Singh.

Eleven politicians who have died of a heart attack are Pandit Jawahar Lal Nehru 74, Zakir Hussain 72, Fakhruddin Ali Ahmed 71, Lal Bahadur Shastri 61, Krishna Kant 75,Dr.BCRoy 80, Dayanand Bandodkar 62, G Ramachandran 70, Sarojini Naidu 70, PV Narsimha Rao 83 and Arjun Singh 80.

Most cine celebrities died of heart attack before the age of 50 and none of the politicians died of heart attack before the age of 50.

Many politicians were assassinated: Indira Gandhi, Rajiv Gandhi, LN Mishra, Mahatma Gandhi, Beant Singh, Phoolan Devi and Abdul Gani Lone.

Only two politicians died of cancer: Shri VP Singh and Chandra Shekhar (blood cancer)

One politician died of surgical complications: Sikandar Bakht and one died of multi organ disease: S. Venkataraman.

No suicidal deaths have been seen in cricketers or in politicians.

 

 

Patients with depression are more likely to have paralysis (stroke) than mentally healthy people and their strokes are more likely to be fatal, according to a study published in the Journal of the American Medical Association.

The researchers looked at data from 28 studies of more than 300,000 people. Over a follow-up period that ranged for two to 29 years, there were 8,478 strokes. Depression was associated with a 45 percent increased risk for stroke and a 55 percent raised risk for fatal stroke. Depression was linked to a 25 percent higher risk for ischemic stroke.

About 3.9% of stroke cases in the US could be attributable to depression.

Lycopene may lower stroke risk in men. Foods such as tomatoes, guava, watermelon, and grapefruit are good sources of lycopene. When tomatos are cooked, the heat processing increases levels of cis-lycopene, which is easily absorbed by the body.

As per a report published in Oct. 9 issue of Neurology, by Jouni Karppi, PhD, of the University of Eastern Finland in Kuopio, compared with the men who had the lowest serum concentrations of the carotenoid, those with the highest concentrations had significantly lower risks of stokes.
